Premier Hospice & Palliative Care provides hospice services to patient's with a terminal illness and a life expectancy of six (6) months or less.
Hospice services are available 24 hours a day, 7 days a week, (including Holidays), if needed, and are provided in the comfort of the patient's home, ("home" may be a skilled nursing facility, group home, assisted living complex, or a private residence).
A team of professionals, including a Physician, Nurse, Social Worker, Chaplain, Certified Nursing Assistant (CNA), and a Volunteer, is assigned to each patient, and will provide individualized care based on the needs of the patient and family.
