Position Title: Connect for Health Desk Advocate
Summary: Connect for Health (formerly known as Health Leads) is an organization that envisions a different kind of health&nbsp;care system: one that addresses basic resource needs, such as food, heat, and clothing,&nbsp;among others, as a standard part of quality care.
Connect for Health understands that a lack of basic resources is often at the root cause of poor&nbsp;health. It acknowledges that medicine alone cannot treat the non-medical factors that&nbsp;impact patients' health, and thus strives to integrate basic resources into healthcare&nbsp;delivery. By providing high-impact services to low-income families, Connect for Health is&nbsp;committed to the fight for health equity.
Our program is located at Hasbro Children's Hospital and Rhode Island Hospital's Center for Primary Care. 
Desk Advocates Responsibilities include:
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Working with clients who phone or walk up to the desk; shifts are between 2 &ndash; 2.5 hours.
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Enrolling patients who walk up to the desk and providing them with in-person assistance;
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Entering provider referrals and screens into databases, calling patients referred or screened, and enrolling new clients; and,
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Conducting weekly phone follow-up with designated clients, while on shift at the desk.
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Coordinate community resource connections.
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Attend weekly advocate development and group follow-up meetings, follow-up with clients outside of work (phone) &nbsp;as necessary. 
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Total maximum commitment per week: 7 hours

Skills & Qualifications:
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Undergraduate students interested in public health, medicine, and social justice.&nbsp; 
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Ability to work independently and in a team based approach.
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Attention to detail is important.&nbsp;&nbsp; 
&middot;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Bilingual (English/Spanish) is a plus.&nbsp;
